Milltown Alliance Primary Care Network is seeking an enthusiastic and compassionate Social Prescriber to join our expanding team, supporting patients across four GP practices.

The successful candidate will work closely with practice teams and wider PCN colleagues to help patients take greater control of their health and wellbeing. You will provide personalised, non-clinical support — connecting individuals with community groups, activities, and services that meet their social, emotional, and practical needs.

This role is well-suited to someone with excellent communication skills, a strong understanding of person-centred care, and a genuine passion for improving health outcomes within the community.

Key Responsibilities

  • Work collaboratively with GPs, nurses, and other members of the primary care team to identify patients who would benefit from social prescribing.
  • Deliver one-to-one support sessions and facilitate connections with appropriate community and voluntary sector services.
  • Conduct home visits where necessary.
  • Maintain accurate, up-to-date records and contribute to regular reporting and evaluation.
  • Build and sustain effective relationships with local organisations, community assets, and stakeholders.

Essential Requirements

  • NVQ Level 3, A-Level, or equivalent qualification
  • Full clean driving licence and access to a vehicle
  • Minimum 1 year of experience in a social prescribing, community, or wellbeing role
  • Strong organisational and interpersonal skills
  • Ability to work independently and manage time effectively across multiple sites

Desirable

  • Previous experience working in primary care or within an NHS setting
  • Familiarity with local community support networks and services

Job Type: Part-time

Pay: £24,000.00-£28,000.00 per year

Expected hours: 15 per week
