Title: Social Prescribing Link Worker (Children and Young People)

Hours: 36 hours per week

Contract: Fixed term until end of May 2026

The main purpose of this role is to deliver holistic, bespoke packages of support to Children and Families in the borough of Oldham.

Social Prescribers will work in a strength-focused way to undertake holistic assessments with children and young people and prepare joint person-centered Action Plans addressing a wide range of underlying issues as led by the requirements of the service.

You will need to carry out specific interventions to support individuals to lead a positive lifestyle, to engage with community-based support and reduce their dependency and involvement with other services.

Social Prescribers will be aligned to one or more of the 5 cluster areas in Oldham and will work with colleagues and partners to develop a place-based offer for Oldham residents – working from a range of community venues including clients' homes.

For this role you will need to be confident in caseload management and the use of electronic recording systems.

Job Purpose

  • To provide Social- Prescribing interventions to improve the health and wellbeing of individuals and households

Principle Duties and Responsibilities

  • Provide Social Prescribing keyworker support to individuals and households, delivering holistic support to address a range of health and wellbeing needs
  • Assist the co production of action plans and provide support in line with relevant legislation, quality standards, best practice and codes of ethics to improve health and wellbeing
  • Apply co-production principles in your work to ensure that the user's voice is represented in service delivery
  • Provide support in a range of community venues to successfully engage people in Social Prescribing support
  • Undertake accurate, timely recording using digital caseload management systems to meet contract requirements
  • Apply the consistent application of all relevant policies and procedures including safeguarding, health & safety and equality & diversity to ensure a compliant service
  • Undertake any other duties to meet service and organisational objectives following consultation with your manager

Person Specification

Experience

  • Demonstrable experience of supporting individuals and households with a range of needs using different interventions
  • Demonstrable experience of planning, monitoring and evaluating your own work
  • Demonstrable experience of multi-agency working
  • Demonstrable success in promoting diversity

Knowledge

  • Awareness of the issues affecting individuals and households and their impact
  • Awareness of safeguarding legislation and guidance, and the boundaries of professional relationships
  • Understanding of Health & Safety legal requirements and best practice
  • Awareness of the principles of social prescribing

Skills

  • Primary skills in engaging, supporting and motivating individuals and households
  • Primary communication skills
  • Primary skills in problem solving
  • Primary skills in assessment of needs and in developing person-centred action plans
  • Primary skills in Microsoft Office and database systems
