**The Role: Care-Coordinator**
The role of Care Coordinator is to ensure our support workers are fully supported, have plenty of time to complete their care tasks and then travel to their client.
The Care Coordinator is expected to support the registered manager with recruitment and retention of support workers and undertake observations such as spot checks and medication competency.
The Care Coordinator is also the person responsible when it comes to dealing with initial queries or concerns from our clients, families and/or other professionals.
It is a role that will push you further forward in your career in care management and give you the opportunity to gain new skills.
You will be joining an established company that consists of a Managing Director / Nominated Individual, a Registered Manager, compliance officer and our team of support workers.
**How will you make a difference?**
Reporting to the Registered Manager; you will be responsible for the effective management and supervision of our support workers and will be supporting the registered manager to ensure that our service is providing an excellent standard of care to our service users and colleagues on a daily basis. You will ensure that compliance with the fundamental minimum standards and using effective rostering to fully utilise staff hours.
- Responsible for the allocation of care workers to service users visits; by using effective rostering, ensuring continuity of care and taking into account locations, travel time, skills required and service user preferences.
- Take initial responsibility for new referral opportunities from both private clients and/or social services, ensuring full information is received and appropriate care planning is undertaken. Assist both the deputy manager and the registered manager with identification of further business opportunities outside of current client base.
- Matching support workers to service users, ensuring support workers are notified of all conditions relating to the care plan.
- You must have good IT, communication and general office skills
- Liaise as necessary with partner professionals and organisations (e.g., social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care
- Maintaining full up-to-date and accurate records and files for all Home Support Workers and Clients in line with statutory requirements and company procedures.
- To ensure that telephones are answered promptly and people are spoken to in a polite and respectful manner
- Dealing with any queries from clients, families, employees and other professionals in a calm and professional manner
- Organising and supporting the registered manager with staff supervisions and appraisals
- Attending and participating in staff team meetings
- Any other tasks required by Registered Manager which she/he deems necessary for the needs of the business
- To share the on-call responsibilities which includes every other weekend
- Delivering care as and when necessary - i.e., covering staff sickness
