Look no further Join our team as a Band 6 Clinical Nurse Coordinator at the Single Point of Access (SPA) Team for Physical Health and be part of a dynamic and innovative healthcare organisation.
As a Band 6 Clinical Nurse Coordinator, you will play a crucial role in coordinating and optimising healthcare services, ensuring that patients in Cheshire West have easier access to hospital and community services. Your expertise and coordination skills will be instrumental in facilitating the management of patients with unplanned care needs, preventing unnecessary hospital admissions, and supporting discharge from secondary care.
**What You'll Do**:
Coordinate and manage the delivery of healthcare services, ensuring excellence in care provision.
Collaborate with healthcare professionals, including GPs and Care Community Teams, to triage referrals and assess patient needs.
Monitor hospital bed-state and manage patient flow to inpatient departments, ensuring timely and appropriate care.
Facilitate ambulance requests for inbound referrals, ensuring safe transportation to healthcare facilities.
Provide general information and support for all patients, including treatment details, home situation, mobility, and community services.
Ensure accurate and detailed information is included in transfer of care letters, particularly for medication administration, wound care, pressure ulcers, catheter care, and palliative/end of life care.
**What You'll Bring**:
✨ Registered Nurse with a valid NMC registration.
✨ Proven experience in a clinical nursing role, preferably with coordination responsibilities.
✨ Excellent communication and interpersonal skills, building strong relationships with healthcare professionals and patients.
✨ Strong organizational and time management skills, prioritizing tasks and meeting deadlines.
✨ Sound clinical knowledge and understanding of best practices in healthcare delivery.
✨ Ability to thrive in a fast-paced and dynamic environment.
✨ Flexibility to work between 8am and 8pm, seven days a week.
