Are you wanting a new challenge?

Single Point of Access are are looking for experienced Nurses who are highly motivated, automonus and have excellent communication skills. You would be joining a well established team of Nurse Coordinators based in Ellesmere Port. This is a non-patient facing role working within a call centre setting.

Planned shortlisting will commence week commencing

Main duties of the job

The post holder will play a key role in the coordination and management of patients with unplanned care needs. They will ensure that patients are directed to the most appropriate service that aligns with their individual health requirements, maintaining a patient-centred approach throughout. Responsibilities include triaging patients via telephone through the Clinical Assessment Service, facilitating appropriate hospital admissions when necessary, and preventing avoidable admissions by making effective use of community services. The role also involves supporting seamless transitions of care across primary, secondary, and community care settings.

  • Registered General Nurse (RGN)

  • Post –registration qualification in relevant subject
  • Previous experience of facilitating clinical supervision
Knowledge and Expertise

  • Evidence of ongoing professional development
  • Excellent communication skills, verbal and written
Experience

  • A Registered Nurse Adult, currently practising with post-registration experience within a community/primary care setting
  • Previous experience of telephone triage
  • Leadership / management skills
  • IT literate with good keyboard skills
Desirable criteria
  • Experience of face-to-face triage / multidisciplinary / interagency working/and clinical streaming
