You will be responsible for booking and managing patients’ journeys, monitoring non-attendance, capturing transport data and attending transport meetings. You will work 37.5 hours a week Monday-Friday.

This is a full-time post.

**Key responsibilities of the role**:

- To ensure the efficient day-to-day operation of patient transport by facilitating and organising the distribution of patient journeys.
- To assist with transport and non-transport administrative and clerical duties to ensure the effective running of the Haemodialysis service.
- To communicate efficiently with patients, transport providers and staff providing accurate information to ensure the correct needs are met for patients and their journey requirements.
- A variety of clerical duties, to help deliver support to the haemodialysis nursing teams and help support to dialysis patients with managing their journeys to and from dialysis.

Customer Care/ Admin duties
- To assist in the maintenance of up to date, comprehensive and accurate records in accordance with current legal requirements
- The role may involve cross cover of the dialysis unit reception, ensuring all patients and visitors are greeted appropriately and given accurate information, ensuring patients’ change of details are accurately entered.
- The role requires the confidential management of patient information; ensuring up to date records are kept on all systems and working as part of a team.
- To assist in the admission and discharge of haemodialysis patients
- To support patients and/ or healthcare professionals during transport to other departments within the Trust
- To assist ward staff with general collection/delivery of items necessary to ensure effective operational management of haemodialysis programme.
- Liaising with the outpatient administrative team to organise patients reviews and other appointments and working with the haemodialysis coordinator to ensure patients reimbursements for expenses are correct and timely.
- This role involves co-ordinating deliveries and collections with couriers and suppliers and assisting the team in organising continued dialysis during patient’s holidays.
- Understand patient mobility, special needs and the importance of booking appropriate transport for each patient.
- Ideally have a good knowledge of local geography

Infection Control
- The post holder has an important responsibility for and contribution to make to infection control and must be familiar with the infection control and hygiene requirements of this role.
- These requirements are set out in the National Code of Practice on Infection Control and in local policies and procedures which will be made clear during your induction and subsequent refresher training. These standards must be strictly complied with at all times.
