**About the Role**

The BAC works in partnership with North Staffs Combined Health Care Trust and Wearewithyou to form the Community Drug and Alcohol Service (CDAS).

The Rough Sleepers Outreach Services Team will integrate into CDAS and further the wider health and care pathway to successfully engage with people, sustain that engagement and improve outcomes of those who are/at risk of rough sleeping including supporting people to access and maintain stable accommodation.

Working alongside the City Council commissioned Rough Sleepers Outreach service and facilitate access to the Rough Sleepers Hope Street Café and homeless hubs in Tunstall and Hanley this outreach post will provide access to wrap around support by:

- Being outreach based
- Providing flexible intensive support bespoke to individual service user needs
- Providing a responsive approach to support people to stabilize in treatment and progress to recovery.
- Providing support to secure accommodation in liaison with the City Council’s Housing Support Solutions Service
- Working in a strengths
- based, outcome focused approach to promote Service Users and Communities to take responsibility for health, wellbeing and recovery.

As **Rough Sleepers Intensive Support Worker, **your responsibilities will include:

- Supporting clients who are in unsuitable accommodation, at risk of imminent homelessness or rough sleeping.
- Providing intensive psychological support.
- Supporting people in treatment and progress to recovery.
- Enabling people to engage in sustained treatment, including access to stable accommodation.
- Encouraging service users and communities to take responsibility for health, wellbeing, and recovery.
- Working within local hostels, supported accommodation, local soup kitchens and care sites to ensure service users that they are informed of their recovery options.
- Motivate service users to take opportunities to achieve sustained recovery and develop clients’ personal strengths.
- Undertake comprehensive assessments of service user’s needs, identifying levels of risk and implementing ways of managing risks.
- Deliver activities and interventions alongside peers and volunteers.
- There is a close working relationship with the community clinical team supporting them to initiate opioid substitution therapy, facilitating increases to support stability and supporting with reductions or referring to inpatient settings where appropriate.

A lead care co coordination role within a multi-disciplinary approach for clients with multiple and complex needs

**In order to be successful you will require**:
Sound working knowledge of substance misuse services and issues surrounding homelessness.
- Knowledge and understanding of harm reduction

Clear communication skills with service users and colleagues.

Self-motivation and the ability to work alone and in a team.
- Knowledge of data protection laws and safeguarding.
- Patience, politeness, and good interpersonal skills.
- Applicants must have a current and valid driving licence and access to own transport to travel to and work from various locations.
- An understanding of the cause and effects of substance use on clients, families and the community.
- Experience supporting service users on a 1:1 basis
- Experience of working in partnership with other agencies to achieve the best outcome for the service user and the “wrap-around” services in the local area.
- Experience supporting service users in centre or community based settings working with Multi-Disciplinary Teams
- An ability to complete assessment of needs, support planning, implementing & evaluating holistic person centred risk / aftercare plans to a high standard.
- Knowledge of the Policies and Procedures relating to Safeguarding of children and adults at risk and how this would be implemented within the workplace
- Ability to motivate, challenge and empower individuals to achieve meaningful and sustainable recovery goals.
