We have an excellent career opportunity for the role of **Care Team Leader** to join the team here at our **Mansfield** Supported Living locations.

**Contract**:42 hrs a week.

**Pay**:£12.75 an hour

**Who is the client?**

Leading Health & Social care provider in the Midlands, providing support to 100s of individuals with learning disabilities, complex needs, autism & mental health needs. We hold contracts with over 20 Local Authorities & CCGs and continue to build relationships with commissioner’s to develop bespoke care facilities.

**Key responsibilities**:

- Supporting young adults and adults with learning disabilities, physical disabilities, complex needs and challenging behaviour within a supported living environment
- Providing high standards of care and support for service users, in line with care and support plans, compliance with CQC regulations and the care & support policies and procedures
- Identifying needs and preferences for each individual and providing personal care
- Encourage good community relationships by promoting a variety of activities
- Accurately report, record and maintain records as required and working to care plans
- Be a kind, empathetic and people person who works well in a team.

- Senior Carer or care Team Leader: 1 year (required)

Licence/Certification:

- Driving Licence (preferred)
