Being a Care Coordinator with Quality Care (SC) is about more than just rotas’ management on highly efficient standards.

It also includes the delivery of excellent care support services, along with assessment and review of individual services and the practical supervision, spot checks, instruction and support of care workers - due to the field based elements of the role you must be a driver with your vehicle.

You will also be required to look for opportunities for service development and business growth.

**Typical Duties**:

- Organise and coordinate weekly, rotas and Care Services.
- Ensure that the care worker rotas are organised and sufficiently well planned taking into account travel time and routes - whilst supporting carers to maintain an appropriate work/ life balance.
- Make sure there is sufficient cover to deliver care and support on time.
- Arrange cover for care assistant sickness, absenteeism, or holidays.
- Carry out assessments, care reviews and risk assessments as required.
- Working with the Registered Manager to implement new care packages on the weekly roster, including identifying the right carers.
- Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care.
- Dealing with queries from clients and families in a calm and professional manner.
- Conducting telephone monitoring and service user visits to ensure a high-quality service.
- Take part in the On-Call Rota (**paid additionally**) and be willing to deliver care, if required.
- Work with the office team to maintain up-to-date electronic and handwritten records.

**Requirements of a Care Coordinator**:

- Minimum of 1 year Experience in Home Care Coordination.
- At least SVQ 3 Qualification.
- Full driving license and owning a vehicle.
- Strong IT and administrative skills.
- Team player, reliable, trustworthy & with high integrity.
- Self-motivated, proactive & uses own initiative.
- Impeccable attention to detail.

**Job Types**: Full-time, Permanent

Pay: £28,000.00-£31,000.00 per year
