Patient Pathway Co-ordinator

The post holder will provide clerical support to the Pathway Coordinator/Medical Secretary, Consultant’s and clinical teams within the department. You will be required to work on your own initiative using knowledge, skills and experience to manage and prioritise workload independently in accordance with operational standards and protocols.
Responsible for supporting the day to day management of the department’s workload, ensuring that an efficient service is provided to the Consultant’s and their teams. Exercise initiative and carry out their duties in such a way as to make a direct and positive contribution to the organisation.
- To provide a full secretarial service to the Clinician/s including transcription or processing of clinic letters (some of a distressing nature), reports and inpatient summaries using EPIC system, ensuring that quality standards are met.
To ensure all patients demographic details (address, postcode, GP & Ethnic Group) are checked and any amendments/corrections are carried out in an accurate and timely fashion.
- To act as a point of contact for users of the Trust, dealing with telephone calls and enquiries promptly and efficiently, providing immediate response and referring as necessary. These will include personal enquiries from patients and their relatives, as well as calls from GP’s, clinicians, students, nursing and administrative staff all times understanding the sensitive nature of calls. To ensure patient confidentiality at all times. There is a need to demonstrate an understanding and reassuring manner when dealing with patients and their relatives.
- To organise the daily office routines, such an opening and sorting post, preparing replies to routine correspondence and photocopying for the team as necessary.
- To initiate, where appropriate, finding the medical records in order for the clinician/s to prepare a reply to correspondence/enquires. To process all patient referrals with nationally agreed time frames, i.e. ensuring 2-week wait cancer policy and in accordance to the Trust’s Access Policy.

An excellent understanding of the 18-week RTT process and knowledge of the Trust’s Access Policy: an agreement to adhere to adhere to policies according to National Guidelines.
To monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies. To identify from the tracking list the next steps in the patient care pathway in line with clinical need. To ensure that pathways are actively managed and any appointments are made in line with the Trust’s Access Policy.
Develop an understanding of the milestones of the allocated pathway for patients within a designated group. To identify and suggest improvements that can be made to the patient care pathway and/or the patient pathway tracking process.
This job description is generic and posts will differ in emphasis depending upon the size of the Consultant’s clinical practice and administrative team therefore this post may develop time to meet the specific needs of the clinical unit.
