The following are the core responsibilities of the Operational Care Co-ordinator Lead
- Care Navigation(CN) Call Centre. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a) Manage the running of CN hub and deal with queries from team in liaison with Senior Assistant Supervisor and Assistant Supervisor b) Work with TLCP operation/hub manager for ensuring efficient service provision. c) Provide coordination and navigation for people and their carers across health and care services, alongside working closely with social prescribing link workers and other primary care roles. d) The Lead will engage/partake in care planning for our patient groups, these will include but not exclusivelyCancer patients who call up, Palliative care patients who call up, House bound patients who call up,Any other patient who the call centre team need Care Coordinator level advice on.

k) Help people to manage their need, answering their queries and supporting them to make appointments l) Raise awareness of shared decision making and decision support tools and assist people to more prepared to have a shared decision-making conversation m) Supervise day-to-day running of CN hub and deal with queries from team and patients n) Support the management team in the compilation of hub reports/audits and the any development plan o) To perform regular system checks to maximise performance p) To ensure staff adhere to practice policies regarding IT security and ensuring conformance to Information Governance q) Performance management of CN hub team with support from operation/hub manager r) Manage CN hub rotas and arrange cover s) Provide guidance in trouble shooting when requested t) Collate required data for reporting purposes u) Monitor and action tasks from doctors v) Be prepared to take on additional responsibilities where reasonable as and when the hub expands to take on this additional workload. Secondary Responsibilities: In addition to the primary responsibilities, Operation Care Coordinator Lead
- Care Navigation Call Centre may be requested to provide additional duties as the hub evolves with changes to TLCP. a. Partake in audit as directed by the audit lead b.

Support administrative staff development, providing guidance and direction, ensuring staff are up to date with mandatory training c. Identify and provide team training where required d. Coordinate and engage in the hub efficiency process/TLCP vision
