Orthopaedic Advanced Clinical Practitioner

The post holder will be responsible for co-ordinating the pathway of care for all trauma patients.
Highly specialist knowledge and advanced skills are required in order to assess the physical, psychological and social needs of the patients admitted following admission.
To work effectively and resourcefully by coordinating the clinical care of the patient and implementing clinical decisions made by the consultant, using expert knowledge and clinical judgement to decide whether to refer patients for investigations and make diagnoses.
Coordinating care to ensure patients are fully optimised prior to surgery by providing expert assistance during the peri-operative phase of patients undergoing orthopaedic surgery. To decide on and carry out treatment, which may include the prescribing of medicines, or investigations and make initial diagnoses.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including the initiation of effective emergency care.
To act as a resource for nursing and medical staff requiring specialist advice.
To provide evidence of effective clinical pathways and improved patient outcomes, this would include developing new and reviewing existing pathways.
Deliver nurse-led follow-up clinics, both virtual and face to face, within the outpatient setting, including advance assessment and clinical examination, referral for investigation and completion of clinical records

The post holder will be an advanced health care practitioner, or working towards this, working within
broad operational and clinical policies and procedures across clinical boundaries.
Monitor trauma surgery demand on a daily basis and liaise with the Operational Team if additional
capacity is required to meet this demand; including surgeon specific cases which may cause capacity
issues in the future.
To work as an effective member of the MDT coordinating clinical care. To participate and lead in
departmental audits of care specific to their area of responsibility and support the education of other
staff by serving as teacher, mentor and supervisor.
The post holder will be expected to integrate theoretical advances and evidence based research
findings into clinical practice, through pathway development and reviews.
To work closely with the Emergency Department and assist in the flow of emergencies through the
department including patients transferred to the Surgical Assessment Unit.
To work collaboratively with other members of the team, including Anaesthetists and Care of the
Elderly in assessing orthopaedic patients to ensure they are fully optimised for surgery. Provide
support to the Ortho-Geriatric ward rounds.
The post holder will be required to exercise a high degree of personal professional autonomy and
make critical judgments of the highest order to satisfy the expectations and demands of the job.
To collate information/patient data that will support the Information Department in providing accurate
data; specifically ensuring all injuries sustained by trauma patients are recorded in full and all cases
meeting the criteria for inclusion in TARN as passes to the TARN data collector.
To assist with the collection of data required for the National Hip Fracture Database; including a
weekly fracture neck of femur audit against best practice guidelines.
The post holder will work their capacity flexibly according to peaks and troughs in demand in order to
aid patient flow. Cross covering other members of the Orthopaedic Nursing Practitioner Team during
period of absence to fulfil an efficient and effective service across all branches of the Orthopaedic
speciality and provide a 52 week service.
Deliver nurse-led follow-up clinics both virtually and face to face within the outpatient setting, including
assessing, onward referral and completion of clinical records.
Delivery of advanced nursing skills, including advanced assessment and physical examination, clinical
decision making, requesting diagnostic investigations and prescribing.
