The Mid Dorset Integrated Community Hub is looking for an experienced Advanced Practitioner to join their multidisciplinary team based in Dorchester. The team consists of Advanced Practitioners, OT's, Physiotherapists, Rehab Assistants and admin support and provides community based support and rehab to the adult population of Mid Dorset.

Mid Dorset is a large geographical area that covers both town and rural village GP surgeries, therefore the role will involve driving around rural Dorset.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

The role will work alongside another Advanced Practitioner to provide highly skilled clinical assessment and management of people in their own homes with the aim of avoiding unnecessary hospital admission, support timely discharge from acute and community hospitals, provide proactive and holistic assessment and care planning for frail older people.

The role works very closely with District Nursing Teams, GP's, the care home frailty team, Social services and other specialist teams to ensure an integrated and enhanced MDT approach for the frail vulnerable population.

The post holder should ideally be a non-medical prescriber and either have completed or be in the process of completing the Advanced Practice MSc qualification.

We are looking for an experienced clinician who can work autonomously as well as part of a team. High level decision making and risk assessment skills are vial as well as the ability to problem solve and communicate effectively with a range of people.
