**Targeted Early Help Locality Team**

**About the role**

**12 month Fixed term contract**

We have an exciting opportunity to join our Targeted Early Help Locality Team as we are recruiting for a full time Early Help Practitioners.

What it's like working in our team

We are a dedicated and fast paced team working across our locality areas. We work closely with schools, Children's Social Care, Health Services and the Third Sector, to contribute to the best possible outcomes for children, young people and their families. Your contribution will have a big impact on what we do, and you will be trusted to work independently and as a part of a team. The work is varied, fast paced, sometimes challenging with the opportunity to develop creative, strength based and innovative solutions.

The role involves:

- Working directly with children and families
- Completing Early Help Assessments
- Formulating plans of support
- Working with multi agency partners
- Leading multi agency Team Around the Family meetings and reviews
- Direct intervention with children and their families

Normal hours of work are 37 hours per week based around the usual business working week of 8:45 17:00. Dependent on service requirements, you may be required to work flexibly outside of these hours to meet family’s needs.
