We are looking for 1 enthusiastic CPN to join our friendly Multi-disciplinary team.

You will require experience of working in a mental health settings and awareness of delivery of community mental health care.

You will hold and manage a caseload of people and have Care Coordinating responsibilities under the Care Program Approach Guidelines.

The team covers an ethnically and culturally diverse area, and you will have the opportunity to work with clients from a wide range of backgrounds, and with a wide range of psychosocial needs. By assessing your clients and their carers needs you will be able to access and sign post to a wide range of community resources and provider services available in the local area.

As we recognise the importance of supporting our staff to achieve an effective work-life balance and also maintain positive wellbeing, we work with staff to develop ways of supporting flexible working for them.

Assessments, care planning, psychosocial interventions, safeguarding, duty assessments.

Report writing, attending tribunal hearings. Supporting with the depot clinic.

Be a full member of the multi-disciplinary team (MDT).

Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures.

Undertake formal mental health and social care/ care act assessments of new clients within own caseload.

Undertake assessments of clients as part of the CMHT duty system in accordance with agreed operational procedures.

Assess, care-plan, provide interventions and evaluate and record community mental health and social care delivered to clients on own caseload.

To provide effective care coordination to an identified caseload of complex service users, ensuring the delivery of evidence based interventions to encourage successful outcomes under the Care Programme Approach (CPA).

Provide Specialist Mental Health Assessments, Continuing care Assessments and advise management plans as requested e.g. from Local Authority Social Workers, Medical, Nursing or Social Workers and nurses from the CCG.

To promote positive mental health and physical care to clients in all situations.

To encourage and support clients to collaborate in their treatment programme and recovery.

To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients.

To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.
To take the clinical lead for the service within your area of specialism and provide advice, guidance and support to colleagues and other professionals.
To undertake specialist assessments when required in line with the Mental Capacity Act 2007, The Mental Health Act and Local Authority Safeguarding Adults Procedures.

To undertake and record comprehensive risk assessments, devise and implement risk management plans for clients at risk of vulnerability, self-harm or risk of physical, sexual or emotional harm to others, using the STAR V 2 Risk Assessment Tool.

To take a lead role in service developments within your area of specialism.
