Job Overview

We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing patient care plans, ensuring seamless communication between clients, healthcare professionals, and support services. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring specialised care, while working within a supportive and professional environment. Prior office experience is desirable to facilitate efficient administrative operations.

Duties

  • Coordinate and monitor individual care plans in collaboration with healthcare providers and clients
  • Schedule appointments, follow-ups, and support services to meet client needs
  • Maintain accurate and up-to-date records of client interactions and care activities
  • Provide occasional hands-on care when absolutely necessary (paid separately)
  • Act as a point of contact for clients, families, and healthcare professionals to address queries and concerns
  • Organise documentation, reports, and correspondence related to client care
  • Assist with administrative tasks such as data entry, filing, and managing schedules using office software
  • Ensure compliance with organisational policies and confidentiality standards
  • Support the onboarding of new clients by providing necessary information and guidance

What We're Looking For

  • Experience as a Care Coordinator, Senior Carer, or Team Leader (preferred)
  • Strong organisational and communication skills
  • Ability to work under pressure and manage last-minute rota changes
  • Flexibility to support the service outside standard hours
  • A caring, patient, and professional approach
  • Full UK driving licence & access to a car (essential)
