Care Coordinator

**Job Summary**
As a Care Coordinator you will work in an office, but may visit clients to ensure effective supervisions and effective care provision are being followed by Field Care Supervisors, Team Leaders and carers.
Hours: 9:00-17:00
**Duties**
- Coordinate patient care activities and services to ensure seamless transitions between different levels of care.
- To schedule carers’ visits and QA tasks to Field Care Supervisors, Team Leaders and Senior Carers.
- To monitor and evaluate all tasks of the Care Worker via channels of staff spot checks, supervisions and evaluation of records
- To assist Field Care Supervisor carrying out spot checks if required
- To set up a new care package by carrying out initial risk assessments and allocating carers ensuring that that new care package hasn’t started without Care Coordinator/Field Care Supervisor/Team leader/ Senior carer first carrying out relevant risk assessments
- To consult with family members and other support to ensure that care is on-going and that all client needs are identified
- To coordinate appropriate care for carers and equipment, including other community resources as required (e.g. OT visits)
- To establish and maintain a current, accurate, confidential client reporting system
- Assist Field Care Supervisor carrying out care plan reviews and communicate with carers to ensure compliance
**Skills**
- Strong organisational skills with the ability to manage multiple tasks efficiently.
- Excellent communication skills, both verbal and written, to interact effectively with patients, families, and healthcare professionals.
- Empathy and a patient-centred approach to care are essential for building trust with patients.
- Ability to work collaboratively within a team environment while also being self-motivated.
- Attention to detail to ensure accuracy in record-keeping and care coordination tasks.
