With the help of the national Local Area Coordination Network, South Tyneside Council alongside its Alliance partner has established a new Local Area Coordination team.

**About the Role**:
The Local Area Coordinator will act as an accessible point of contact for people and families in a local area. Any local person will be able to connect with them with no referral needed and no time limit to the relationship. The Local Area Coordinator’s job will be to connect with and “walk alongside” people and families helping them find sustainable (often non-service based) solutions to any concerns / challenges, whilst also promoting inclusive connected communities too. Most of a Local Area Coordinators role will be spent alongside people and families experiencing some form of exclusion and complex challenges, helping them envision and coordinate their plans for a better life.

This varied and dynamic role includes:

- Helping people access personalised information and short-term support in their local area.
- Developing longer-term relationships with people/families facing more complex life situations. Local Area Coordinators dedicate time to helping people recognise their strengths, explore options, make choices, plan for the future, and access sustainable solutions.
- Cultivating strong partnerships with community members, groups, agencies, and services to support local community capacity building and closer collaboration.
- Collecting stories and information to drive transformative changes in the wider health and social care system, showcasing how the Local Area Coordination approach and principles can reduce need for statutory, funded supports, and commissioned services.

**About you**:
You will have outstanding listening, communication and interpersonal skills for building trusting relationships. You mut be committed to not ‘do to’ or ‘fix’ people and instead help people experiencing complex issues to identify and pursue their own goals using evidence-based approaches.

You will be experienced in supporting people and families from diverse backgrounds with person-centred and holistic approaches (for instance Health Literacy and Co-production) to build a picture of the life they want for themselves and supporting people to create plans towards achieving their vision.
