This is an exciting opportunity to be part of a multi-disciplinary Older Adult's Home Treatment Team, which is clinically managed by a Senior OT. It is a friendly and supportive team and we welcome new staff and the ideas and skills they bring.
The HTT works intensively with people with dementia and people with acute metal illnesses and age related frailty. The service is looking for a Band 5 Occupational Therapist who is passionate about using their skills to work with older people experiencing a mental health crisis and those who have dementia.
The service has an intense interface with psychiatric liaison services and inpatient services, providing gatekeeping assessments authorising admissions as well as facilitating early discharges; making this an ideal opportunity for an enthusiastic practitioner who wants to work and develop their skills in a well-established team and influence how care is delivered for this client group.

The post holder will have both generic responsibilities and Occupational Therapy specific work to carry out in the community. As an independent practitioner in SAMH Home Treatment Team you will be supported to deliver care that enables service users to remain in their own homes during a period of mental ill health. Training opportunities will also be provided to enhance your knowledge around dementia and age related frailty.
The post-holder will be expected to assess and deliver, evidence-based OT interventions, to facilitate wellbeing, and optimise independent functioning in self-care, productivity and leisure activities, from an occupational perspective. Interventions take place in home environments and community settings with flexibility of working required to meet service user needs.
There is an expectation to be involved in projects or service developments happening within teams allowing the post-holder to develop their skills within this area.
CPD for Band 5 occupational therapy staff includes a preceptorship programme, band 5 support group and in-service occupational therapy specific training days. There is an active CPD programme which includes regular professional supervision and appraisals, dedicated CPD time and opportunities to be involved in quality improvement projects.

Occupational Therapy specific
To provide specialist occupational therapy assessment and intervention in the service users’ home to individuals living with dementia, mental illness and age-related frailty who are experiencing mental health crisis and in the absence of the team may require hospital admission.
To work in collaboration with service users, referrers, colleagues and carers to formulate and deliver intervention plans that augment existing care and aims at reducing the disabling impact of mental health crisis. To identify OT goals as part of the overall care plan.
To plan and implement individual and/or group interventions in collaboration with service users using graded activity to achieve therapeutic goals.
To assess for and provide equipment including instructions on safe use to maximise independence and quality of life as well as advise on environmental adaptations and use of assistive technology where indicated or refer on to other relevant agencies when appropriate.
To monitor and evaluate treatment, to measure progress and ensure the effectiveness of intervention.
To provide the occupational therapy perspective to MDT meetings and case discussions including at point of discharge and or transfer of service users.
To be a resource for the team providing occupational therapy professional guidance, and to lead on the evaluation and continued development of the occupational therapy service within the team.
To comply with the RCOT Code of Ethics and Professional Conduct and national and Trust policies and procedures.
