Geographically it covers North Staffordshire, Stoke, East Staffordshire Southeast, Staffordshire & Seisdon Peninsular, Cannock Chase and Stafford & Surrounds.

The Case Management team works within the Trust to support the delivery of Transforming Care across all Trust Divisions, working to ensure that people with learning disabilities and / or autism are supported to lead healthy, valued lives.

The purpose of this role is to support the commissioning of safe and effective services for people with a learning disability. This supporting the discharge from hospital and the oversight of commissioned community packages of care thereby preventing hospital admission.

It will involve participating in the assessment, planning and implementation of pro-active care and case management for the population of Staffordshire and Stoke
- on
- Trent with a Learning Disability and or autism. The post holder will be the main point of contact for health and social care colleagues as well as providers, service users, family and carers in relation to the appropriateness of placements and facilitating the care system to move people through the system with regards to the Transforming Care Concordat.

The post holder will be a member of the Learning Disability Transforming Care Team within the Specialist Directorate of North Staffordshire Combined Healthcare NHS Trust. They will be accountable to the Service Manager and will have designated responsibility for the co-ordination and delivery of the Transforming Care Programme.

1.1 Work as part of the team within its day-to-day activity ensuring that plans and targets are fulfilled.

1.2 Ensure that the role is managed in line with related Trust policies and that any failings are reported and corrected accordingly.

1.4 Ensure contribution as required to the delivery of performance development and quality improvement programmes.

1.5 Ensure commitment to Trust appropriate support, supervision and development in line with related Trust policies and specific to these specialist teams.

1.6 Ensure that activity supports efficient use of resources and is consistent with the teams allocated budget, supporting the Service Manager in ensuring the overall Service budget is managed effectively.

1.7 Develop and sustain relationships with others that create a sound base for communication, services delivery and partnership working as required.

1.8 Monitor use of supplies/equipment and stock levels within the department, ensuring cost effective use of resources.

1.9 Keep abreast of changes and developments in national policy and legislation pertinent to the service area and attend in-service training and courses as required. Promote a culture of learning and personal development for staff by leading by example.

1.10 To promote practice within professional codes of conduct/practice, providing an environment where knowledge and expertise are maintained and developed within the scope of professional practice.

1.11 Compile and complete statutory documentation where appropriate.

1.12 Keep appropriate records with an awareness of legal requirements with respect to record keeping and the use and transfer of information which balance and manage the competing requirements of confidentiality and effective information sharing to the benefit of the client and carers.
