An exciting new opportunity has arisen within the Medicine Care Group for a Coordinator supporting the Neuro-disability service.
In this role, you'll collaborate closely with clinical and management teams to ensure appointment schedules are well-organised, clearly communicated, and accurately documented. You'll also be responsible for ensuring all follow-up actions are completed efficiently.

**About You**:
Educated to GCSE level (or have equivalent experience), you will be able to
Support clinical teams with administration to help maintain timely patient care pathways.
Reporting into the Operational Manager, your responsibilities will include:
Maintain patient records and databases
Track appointments and clinical outcomes
Minute clinical meetings and ensure follow-up actions are completed
Handle complex queries and triage referrals
Liaise with clinical colleagues to support smooth patient pathways
