We are expanding and now looking for a super hero care cordinator to join our amazing team.

If you can provide high quality home care services that support the rights of Clients to live the lives they chooseas far as they are able and to be responsible for the continuity of care for Clients, ensuring all visits are covered by the appropriately skilled Caregiver - you are the person we are looking for.

Your Main Duties
- Develop Client care plans and re-organise them as necessary when difficulties arise.
- Check on the care that Clients have received and making sure this is recorded and documented accurately.
- Work alongside the Caregivers to make sure the right skills and experience are matched to deliver each customer’s care and support plan.
- Talk to Clients and their chosen representatives about their care and support taking account of different communication needs and levels of understanding
- Work with the wider company team and other health and social care professionals to deliver high quality
- Promote the rights of each Client and keep their wishes at the centre of their care and support.
- Take part in the out of hours emergency on-call rota as and when required.

What we are looking for:
- Somebody with a positive attitude, who wants to grow and develop with the company, and has experience of the role.

Experience of Access People Planner and Care planning would be an advantage.

**Salary**: £18,000.00-£21,000.00 per year

**Experience**:

- providing care coordinator: 1 year (preferred)
- supervisory: 1 year (preferred)

Licence/Certification:

- Driving Licence (required)
- NVQ Level 2 Health & Social Care (preferred)
