Senior Discharge Co-ordinator

Qualified and Registered Nurse, Social Worker or Therapist to work within the Discharge Team at Denmark Hill to provide discharge planning and case management support for patients being discharged on Pathways 1 and 3.
We are seeking to recruit a qualified Nurse, Therapist or Social Worker into a Band 7 Senior Discharge Co-ordinator post at Denmark Hill to join our well established team supporting the vulnerable patient group who require complex discharge planning across the medicine, surgical and Neuroscience wards mainly. This role supports the Trust's Inpatient Flow transformation programme but most importantly the patients being discharged on Pathway 1 and 3.
You will carry a caseload of patients providing support and expert advised to them and their families and you be allocated to support a ward or wards so will need to establish and built a strong working relationship with the MDTs. You will be seen as an expert knowledge base providing support for timely but safe discharge, including offering guidance, signposting, peer review and training. For Pathway 3 discharges you will be responsible for completing and submitting the Trusted Assessor report for care home placement plus you will help with the coordination of Fast Track paperwork completion and submission.
Initially this post will work Monday to Friday, but weekend working may be required in the future. It is patient facing and will require you to attend the wards and you would be expected to attend the daily boardounds.

Support ward based multi-disciplinary teams in making timely referrals and progressing discharge arrangements, addressing areas of concern and offering advice and practical support to enhance discharge plans. To support the ward teams in developing processes to ensure all patients have a discharge plan developed by the ward within 24 hours of admission. Lead on liaising with families in integrated discharge planning, ensure they are kept up to date and organising family, Best Interest and discharge planning meetings as appropriate.
