We'd like you to join us as a Referrals/Discharge Co-Ordinator at our Blackheath Rehabilitation Centre.

Blackheath is a 36 bed centre providing specialist person-centred rehabilitation, care and support for adults with either a brain injury or other complex neurological condition. We provide comprehensive assessment, rehabilitation and therapy and specialise in the treatment of patients with a primary cognitive or behavioural impairment resulting from a brain injury or other neurological condition.

**What you'll be working**:
We're flexible, so you'll work a total 37.5 hours per week Monday - Friday

**What you'll be doing**:
The post holder will act as a referral and discharge coordinator for Blackheath Brain Injury Rehabilitation Centre. This implies acquiring familiarity with the referral electronic system, liaison with referring hospitals, CCGs, social services and NHSE, collation of relevant paperwork and organisation of the assessments and discharge paperwork and administration tasks for discharge ie CHC checklist and working with the social worker to support discharges.

To ensure the smooth running of the unit by supporting the management team and effective line management of non-clinical staff.

To provide an effective administrative and secretarial service to the multi-disciplinary team.

To ensure the effective administration by liaising with key players (i.e. Hospital Director, Medical Director, Ward Managers, etc.) as directed.

Provide pro-active co-ordination of admissions and discharges throughout the units, working closely with the Centre Team Management.

Be the first point of contact for the unit referrals and able to support and coordinate the initial clinical triage with direction from the Medical Director.

Respond to and prioritise referrals.

Chair Admission, Discharge and Transfer meetings and attend Clinical Team Meetings and present referrals for discussion in ADT.

Ensure we meet all targets for outputs, timescales and quality.

To help co-ordinate discharge planning for patients with complex needs, taking responsibility as appropriate for liaising with external agencies to contribute to safe and effective patient discharges.

To contribute to the MDT Building and sustaining strong relationships with the range of community and primary care services, including social services, intermediate care, community matrons, voluntary sector and private sector to contribute to patients receiving a safe and effective discharge plan.

To use the information provided by the multi-disciplinary team to help the patients through the discharge pathway.

Liaise where appropriate with the external agencies such as Social Services and Continuing Health Care team, ensuring that patient referrals and documentation are received in the required time scales.

To give verbal updates to commissioners, health and social care staff and other appropriate professionals on the patient’s progress towards discharge.

To give regular updates on progress and discharge planning to relevant agencies involved in funding or delivery of care on discharge, including CCGs, CHC and Social Services.

To attend patient review meetings or discharge planning meetings if required to facilitate a smooth discharge process for patients.

Take responsibility for ensuring effective communication between all service providers across the discharge pathway.

Escalate potential funding issues to the Hospital Director a month prior to the end of the patient’s 180 day inpatient stay, if they are likely to require further rehabilitation.

To compile comprehensive discharge documentation and disseminate to relevant parties involved in the patient’s care demonstrating appropriate information governance.

Have a working knowledge of all policies and guidelines for the unit and company relevant to your position.

Understand and comply with the policies of the company in particular observing the confidentiality of information pertaining to patients, colleagues, and the business of the company.

**Key Accountabilities: Unit Administrator**

Provide administrative service to the unit manager when necessary.

Accurate filing and maintenance of archives in compliance with legislation.

Assist the designated line manager in processing enquires and/or admissions and maintaining funding for in-patients.

Maintain a high standard of confidentiality and always take a professional approach.

Provide a service to the unit including:
Preparing letters as requested

Maintaining and populating management reports

Provide ‘Front of House’ hospitality/reception services

Coordinate and manage external professional visits to the service.
