**The Role**:
- The post holder will work with peers within the health hub to deliver an efficient service which helps in the achievement of required outcomes. The post holder will assist the service to support targeted residents to achieve long-term changes to their health behaviour.- The post holder will ensure that referrals are processed within 48 hours and in line with the patient pathways and service protocols.
- The post holder will support the hub to ensure that the phone lines are manned 9am-5pm Monday to Friday and that 100% of data is captured.-
The post holder will ensure that all data is entered accurately to ensure excellent standards are maintained.
-The post holder will work with in a team of Administrators/Health Advisors to provide high-level administrative support to the service delivery function and to the field-based team members.
**Key responsibilities: -**Ensure that exceptional level of customer care is maintained through the delivery of a quality service with a strong focus on customer service.
- Screen all referrals for eligibility and their motivation to commence the service using motivational interviewing techniques.
- To receive and process new referrals, send out appropriate documentation to clients and referrers as part of the administration hub.
To process referrals adhering to protocols given by the patient pathways and service protocols within 48 hours.
To participate in all roles of the coordination process to ensure referrals are uploaded in line with the service KPIs.
Complete the entire administrative process for the service, including processing of referrals, data entry, client follow-ups and communication to referrers.
To utilise Pharmoutcomes, My Smoke Free Life, My Information Genius and any IT system deemed necessary to ensure Invoices are coordinated, collated and approved for the Primary Care Contract payments and sent to Morelife Finance team for Processing.
Ensure complete and accurate information is provided to central MoreLife Finance team to enable the payment of invoices for the Primary Care Contract and recommend/approve invoices for payment.
Ensure complete and accurate records are maintained for the Primary Care contract (e.g. signed contracts, data-sharing agreements and active/inactive providers and claims).
