Monday - Friday, 9am to 5pm (flexible hours and working location considered)

You will be expected to work a minimum of 37.5 hours per week, excluding lunch breaks. The role may include evening and weekend hours.

**Location**

Blackburn Rovers Football Club, Ewood Park, Blackburn, BB2 4JF. With travel across the wider Blackburn with Darwen area as required.

**Overall purpose of the Job**

Through the delivery of sessions, workshops and targeted 1-2-1 interventions, this crucial role will work in partnership to collaborate closely with the local authority teams, secondary schools and multi-agency partners to enhance engagement and outcomes for care experienced young people in Blackburn with Darwen and surrounding areas, as needed.

The Targeted Youth Mentor will play a vital role in supporting care experienced young people under the age of 19 who need targeted intervention to work towards reaching their full potential by using a person cantered approach. The Targeted Youth Mentor will work to increase social connectedness, increase physical activity, improve mental health and financial stability, improve personal safety and support the development of social networks.

Supporting young people to build confidence, resilience and pathways into positive futures. Building independent living skills such as money management and budgeting, cooking, alongside guidance to build resilience and to reduce the risk of exploitation. Raising aspirations by offering pathways into further education, training, and employment. Providing wellbeing activities, helping young people to develop resilience, strengthen self-belief, and enhance their overall wellbeing.

This position involves delivering informal education, PHSE, physical activity sessions, workshops, and focused one-on-one interventions. Providing guidance to young people facing specific challenges including emotional health and wellbeing, physical, social and academic difficulties. The role involves building trusting relationships with young people, facilitating personal development and promoting positive life choices.

All staff members are expected to support other Trust areas when and where business need requires to do so.

**Duties and Responsibilities**

- Develop an in-depth knowledge base of support, community activities, and personal development opportunities for young people in and around the Blackburn with Darwen area.
- Develop a good knowledge of local services to support young people’s mental and physical health needs.
- Develop a strong awareness and understanding of when it is appropriate or necessary to refer people back to other professionals/agencies, when what the person needs is beyond the scope of the Targeted Youth Mentor role - e.g. when there is a mental health need requiring a qualified practitioner.
- Maintain/strengthen existing or new professional relationships with stakeholders such as local authority teams responsible for support care experienced young people. Children’s Social service, local secondary schools, Lancashire Constabulary, Blackburn with Darwen Youth Offending Team and others to identify and work with young people at risk, developing strong referral pathways.
- Attend relevant meetings, becoming part of the wider network, giving information and feedback on social prescribing.
- Recommend and review referrals from referral partners and support them to make appropriate referral into BRCT or external programmes.
- Build trusted relationships with young people to promote personal development, wellbeing gains and reduce their experience of offending and antisocial behaviour.
- Ensure all necessary participant details are recorded and used to inform risk management.
- Mentor to provide wrap around support, including signposting to other agencies/services where required.
- Plan and deliver a range of activities to engage clients and support a positive change in behaviour.
- Help young people identify and understand the wider issues that impact on their health and wellbeing, such as debt, poor housing, loneliness, and caring responsibilities.
- Work with individuals to co-produce a simple personalised action plan - based on the person’s priorities, interests, values and motivations - including what they can expect from the groups, activities and services they will be connected to and what the person can do for themselves to improve their health and wellbeing.
