An exciting opportunity has arisen to join the Islington Transitions Team as Clinical Team Lead. The team is a non-case-holding team, comprising of CAMHS and AMHS clinicians, who advise on transitions from CAMHS to AMHS and where young people (18-24) present with complex needs, are not engaging with mainstream services and require innovative care packages.

The Clinical Team Lead chairs the fortnightly transitions meetings, conducts audits and evaluations, and contributes to service developments in the growing area of Young People's Services.

Specifically, we are looking for clinicians who are already embedded within C&I or one of our partner CAMHS agencies who can help us strengthen links with existing services.

To provide a Core Mental Health Psychological Therapist service to the Islington Transition team and to act as team leader.

The Transition Team is a small team of senior clinicians recruited internally from Adult Mental Health Services (AMHS) and from Child & Adolescent Mental Health Services (CAMHS, Whittington Trust) in the London Borough of Islington.

The team recently trained in Applied Mentalization-based integrative treatment (AMBIT) and the team lead is expected to promote and embed the model in the team and when collaborating with other teams/ agencies as part of the implementation of the Community Mental Health Framework.

Requests for help in facilitating the transition of young people from child to adult mental health services come to the Team from CAMHS, AMHS and from some related Local Authority Teams, which could include Leaving Care, young people’s Substance Misuse service and specialist staff working with 16+Children in Need.

The Transition Team does not hold or take case responsibility but rather seeks to support complex transitions by providing advice, consultation and assessment.

The effectiveness of the team is evaluated by audit and outcome measures for evidence of improved transition for young people.

CLINICAL RESPONSIBILITIES
- To develop and provide a clinical consultation, advice and support service for mental health staff within CAMHS and AMHS, where client issues are complex and meet the thresholds for joint interventions and span the border between CAMHS and adult services.
- To provide clinical problem-solving expertise, particularly in the areas of Transitions (CAMHS to AMHS), Emerging Borderline Personality Disorders (EBPD) and co-morbid presentations in young people.
- To promote and embed the AMBIT approach in the team
- To chair the new multi-agency Islington Transitions Team meeting
- To provide clinical supervision of assistant psychologist
- To provide advice, consultation, training and supervision, where appropriate, to other health and social care staff working with the client group.

- and post-qualification teaching of clinical and/or other applied psychologists, as appropriate.
- To participate in the delivery of training and information sessions to colleagues, other services, senior management forums and any other relevant forums, as required.

OPERATIONAL RESPONSIBILITIES
- To provide clinical leadership using AMBIT principles in the Transitions Team
- To coordinate the provision of consultation to professional networks and clinicians working with young people
- To be involved, as appropriate, in the short-listing and interviewing of other posts

POLICY / SERVICE DEVELOPMENT
- To monitor and audit service provision, ensuring practice meets best professional and evidence-based standards.
