Team Leader Integrated Discharge Service

The purpose of the role of the Discharge Liaison Manager is to provide day to day leadership and responsibility for the Integrated Discharge Team of Discharge Nurses, Discharge Co-ordinators and Administration Staff. The team support and co-ordinate discharges, to promote excellent standards of discharge practices throughout Cardiff and Vale UHB to improve the quality of the patient experience. The post holder will also work in close conjunction with the Senior Nurse for IDS, Head of Integrated Care and work in close partnership with Clinical Leads, Social Services and Third Sector and Voluntary Managers, working to develop new models of service delivery in line with Welsh Government (WG) policies and strategic direction.

This post requires a motivated, dynamic and innovative leader. As a Discharge Liaison Team Manager, you will be leading and supporting your team through very challenging times.

An exciting secondment opportunity has arisen for a Band 7 Team Manager within the Integrated Discharge Service for 12 months. The post is multi-site to ensure the service is supported throughout the UHB.

The post holder will manage the Discharge Liaison Service to help support and co-ordinate discharges, to promote excellent standards of discharge practices to improve the quality of the patient experience.

The post holder will work closely with other IDS Team Mangers, Social Workers from both Cardiff and Vale localities, First Point of Contact Officers, Discharge Support Officers and other partner organisations.
The post holder needs to be a knowledgeable resource within the UHB in the field of Discharge Planning, Continuing Healthcare, Delayed Transfers of Care and National and Local policies. To support integrated professional working to achieve best provision of care for service users.
The post holder will be required to have a flexible approach to the needs of the service and team. An exceptional understanding of hospital discharge procedures and experience in working in a multi-agency setting is essential.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
