**Purpose...**: YOU will support CARERS during inpatient periods using your empathy, knowledge and/or professional or lived experience of caring and health and social care. Carers registered with our Charity will rely on YOUR guidance and a listening ear, YOUR wellbeing support, organisation of peer support opportunities, help with planning for the future related to their care responsibilities and introduction to preventative services. YOU will also raise awareness of the challenges that unpaid carers face and provide access to appropriate support for carers within West Sussex by liaising with health and social care professionals within the acute and community hospitals as part of the multi-disciplinary team (MDT).**Before you keep reading**: PLEASE DO NOT SEE EVERYTHING IN THIS JOB AD AS "MUST HAVE", BUT RATHER A GUIDING LIST OF WHAT WE ARE LOOKING FOR.
