We are looking for an experienced and Passionate Peer Mentor to work as part of our Criminal Navigation Service in the North East.

This is a Field based role across the North East of England

How you will make a difference
- I will provide support to individuals being released from the North East Prisons, supporting people post release in line with service guidelines, working alongside other agencies to improve health outcomes and reduce reoffending
- As needed, I will provide support to those being released from court, and provide assistance with these processes
- I model excellent professional boundaries, proactive, positive and solution focused approaches in my work embodying the company values
- I am enthusiastic, passionate and motivated to provide high quality, seamless support for those leaving custody, preventing a return to ill health, reducing health inequalities and reoffending, whilst recognizing the challenges this work can bring
- I support health promotion, planning and delivering health promotion events in collaboration with colleagues
- I complete, detailed, accurate, professional and timely notes for each service user, and upload them to the relevant systems (following training).
- I provide a coaching framework to empower individuals with independent management of their release and associated healthcare needs (training provided).

- I have experience of supporting individuals.
- I have knowledge of health, debts, social care, housing, court systems, mental health, criminal justice and substance misuse.
- I am passionate and motivated to support individuals leaving the criminal justice system through provision of a high quality, professional level of care, recognizing and respecting individual differences and promoting diversity
- I value the unique life journey each individual has been on, and embrace the challenges this can bring
- I am patient, calm and respectful within my work
- I am committed to developing my knowledge and skills in coaching, and am prepared to complete training within the role to support this
- I am experienced in maintaining boundaries, even
