Role: Social Worker – Next Steps Team Department: Children's Services, Durham County Council Location: Durham Purpose of Role Social Personnel is advertising a Social Worker role for Durham County Council. The Next Steps Team is a newly established service supporting young people aged 16‑17 as they prepare for adulthood. The Social Worker will work alongside young people to develop Pathway Plans, set aspirational goals, and ensure a smooth transition into care leaver services. Key Responsibilities Manage a caseload of young people aged 16-17, providing consistent support and guidance. Work collaboratively with young people to co‑produce Pathway Plans that reflect their aspirations and needs. Support the transition from care to independence, ensuring access to appropriate services and opportunities. Liaise closely with the Care Leavers Service and Children in Care Service to provide continuity of support. Utilise wraparound services such as Staying Close, Peer Mentoring, and emotional wellbeing support to enhance outcomes. Maintain accurate case records and ensure compliance with statutory requirements and council policies. Essential Skills & Experience Qualified Social Worker registered with Social Work England. Experience of working with young people in care or preparing for independence. Strong knowledge of legislation and guidance relating to children in care and care leavers. Excellent communication, relationship‑building, and planning skills. Ability to work collaboratively within a multi‑agency environment. Desirable Experience of Pathway Planning and supporting transitions to adulthood. Knowledge of local services and support networks available to care leavers in Durham. #J-18808-Ljbffr
