Job Summary

The Care Coordinator plays a vital role in ensuring that patients receive the highest quality of care by coordinating various aspects of their treatment and support services. This position involves liaising with healthcare professionals, patients, and their families to facilitate effective communication and ensure that care plans are executed efficiently. The ideal candidate will possess strong organisational skills and a compassionate approach to patient care.

Responsibilities

  • Coordinate patient care by collaborating with healthcare providers, specialists, and support staff to ensure comprehensive treatment plans are followed.
  • Schedule appointments, manage patient records, and maintain accurate documentation in compliance with healthcare regulations.
  • Act as a point of contact for patients and their families, providing information regarding treatment options, resources, and support services available.
  • Monitor patient progress and follow up on care plans to ensure adherence to prescribed treatments and interventions.
  • Assist in the development of care plans tailored to individual patient needs, taking into account their medical history and personal circumstances.
  • Facilitate communication between patients and healthcare teams to address any concerns or questions regarding care.
  • Support office operations by managing administrative tasks related to patient care coordination.

Experience

  • Previous experience in a healthcare setting or office environment is essential.
  • Strong organisational skills with the ability to manage multiple tasks effectively while maintaining attention to detail.
  • Excellent communication skills, both verbal and written, with a focus on building rapport with patients and professionals alike.
  • Proficient in using office software applications for documentation and scheduling purposes.
  • A compassionate approach towards patient care with a commitment to enhancing the patient experience.

Job Type: Part-time

Pay: £25,100.00-£25,200.00 per year
