Overview

We are seeking a dedicated and experienced Care Manager to oversee the delivery of high-quality care within our home care services. The ideal candidate will possess strong leadership skills, a compassionate approach, and extensive knowledge of senior care, including complex care. This role involves managing care plans, supervising staff, and ensuring compliance with healthcare standards to promote the well-being and dignity of our service users. The Care Manager will play a pivotal role in fostering a supportive environment that prioritises personalised care and continuous improvement.

Responsibilities

  • Lead and supervise care staff to ensure the delivery of exceptional complex and home services.
  • Develop, implement, and regularly review personalised care plans tailored to individual need.
  • Oversee medication administration in accordance with legal and organisational guidelines, ensuring safety and accuracy.
  • Coordinate with healthcare professionals to facilitate comprehensive nursing and senior care services.
  • Monitor service users health status, respond to emergencies promptly, and liaise with families regarding care updates.
  • Ensure compliance with all regulatory requirements, health and safety standards, and organisational policies.
  • Manage staff rotas, organise training sessions, and support professional development initiatives.
  • Promote a positive environment that encourages independence, dignity, and respect for all residents.
  • Maintain accurate documentation related to service users care, staff records, and operational procedures.

Requirements

  • Proven experience in senior care settings such as care at home.
  • Strong background in nursing or healthcare management with supervisory experience in a care environment.
  • Knowledge of patient care best practices.
  • Experience in developing and managing personalised care plans.
  • Leadership qualities with the ability to motivate and supervise a diverse team.
  • Excellent organisational skills with attention to detail in record keeping and compliance documentation.
  • Compassionate approach with excellent communication skills for liaising with residents, families, and healthcare professionals.
  • Relevant qualifications in nursing or healthcare management are highly desirable.
  • Ability to work under pressure whilst maintaining high standards of care.

Job Type: Part-time

Pay: £11,232.00-£12,480.00 per year

Expected hours: 16 per week
