The role of the Enhanced Care Health Care Support Worker is to support recovery and provide care to those people who have enhanced care needs across the organisation. E nsu ring the care e x pe r ien c ed i s p o si t i v e and that the person's sa f e ty i s m a in t aine d.

To deliver enhanced care across wards and departments to patients who have been assessed as needing additional support on the wards due to dementia, delirium, mental health conditions, challenging behaviour or physical health conditions which are compromising the safety of the patient and/or others. Enhanced care is an integral part of a therapeutic plan and ensures the safe and sensitive monitoring of a patients physical and psychological wellbeing, whilst building therapeutic relationships. Enhanced care could be supporting a group of patients or patients who only require one to one support.

The post holder is expected to t r ea t everyone e.g. patients , their r ela t i v e s, visitors, c ol l eag u e s etc, w i th r esp e c t, di g ni ty, c o u rt e s y and i n accordance with the T r us t's V alu e s.

Main duties of the job

Demonstrate an u n d e rst a nd i ng of the care a n d treatm e nt prog r ammes of people with enhanced care needs, to e n ab l e instruction, a d vise & guidance for patients , fam il y a n d carers. T o make the app r opr i a te a d justm e nts in d e p en d ent l y (within own competence and skill set) and be a b le t o comm u nica t e a n y changes to patients and registered health care professionals .

Develop and maintain meaningful therapeutic relationships with people. Adopt a polite, positive and empathetic approach, communicating effectively and informatively. Assess situations, identify and resolve potential problems, within own competence and skill set. Assist in dealing with verbal/physical aggression using de-escalation techniques.

To provide one to one support and / or supervision for a people/ group of people u nde r t h e s upe r v i s io n/direction o f r e gi s t e r e d h e al th ca r e p r o f e s si o n a ls, within a delegated framework for agreed tasks. Organise a range of activities e.g. facilitate meaningful activities for individuals and liaise with the multi-disciplinary team to co-ordinate patient specific activities and daily plans, in line with risk assessments. E xercise a degree of in d ep e nde n ce, initiative and judgement and consider different approaches when delivering tasks .
