Job Summary

Main Responsibilities:

  • Provide safe and compassionate care and support that is person-centred.
  • Perform personal care tasks as outlined in the client's care plan.
  • Create and maintain good communication with clients and other members of the support network.
  • Establish and maintain effective working relationships with clients.
  • Document, monitor and report any changes to care managers.
  • Assess care plans versus actual provision.
  • Assess risks and needs, reporting to the care manager.
  • Participate actively in clients' care team meetings.
  • Adhere to all Eclectic Care Ltd policies and ensure new staff are inducted into clients' care packages and follow procedures.
  • Offer companionship and emotional support to enhance the well-being of clients.

Ideal Candidate Must Be Able to Support Clients With:

  • Attending appointments and social activities.
  • Helping with dressing.
  • Laundry and light cleaning.
  • Meal preparation.
  • Medication management.
  • Personal care, including washing and bathing.
  • Shopping.
