Summary
We are seeking a Care Coordinator to join our team in the Worcester. Based in our office, the Care Coordinator will play a vital role in organizing and managing homecare services, ensuring our clients receive the highest standard of support. Responsibilities
- Coordinate and schedule care plans for clients to meet their individual needs.
- Communicate effectively with clients, families, and healthcare professionals.
- Maintain accurate records of care activities and client information.
- Assist in managing care staff schedules and assignments.
- Ensure compliance with health and safety regulations.
- Support quality assurance processes by monitoring service delivery.
- Contribute to continuous improvement initiatives within the agency.

- Qualifications
- Proven experience with office administration or coordination roles.
- Strong organizational and communication skills.
- Ability to work independently and as part of a team.
- Knowledge of healthcare or domiciliary care services is advantageous.
- Proficiency in using office software such as MS Office.
- Compassionate attitude with a focus on client well-being.
- Ability to handle sensitive information with confidentiality.
- Prior experience in care coordination or social care is preferred.
