We offer a £2000 welcome payment for qualified and experienced Social Workers/Occupational Therapists (grade J and above), Practice Consultants and Team Managers new to Norfolk County Council. Amount to be repaid in full should they leave within two years of employment.

For experienced Social Workers new to Norfolk County Council we promise to try and match or better your existing salary, providing its in the spectrum of the grade advertised.

An exciting opportunity has arisen for a Social Worker to join the East Place Home First Team, supporting acute hospital discharges as well as developing a new admission avoidance service (Same Day Emergency Care)

Our aim is to embed both Discharge to Assess and Admission avoidance as part of our Ageing Well programme. East Place have already made great strides over the last few years and now need to take forward further rapid redesign in line with national policy and quality improvements to ensure adults entering the 'Front Door' (Accident and Emergency) of James Paget Hospital or being discharged from the Hospital and Community Hospitals, are supported to return home, access reablement opportunities and plan their ongoing care and support needs once discharged.

The Homefirst Team have been working towards an integrated care system, alongside East Place; Primary Care Hubs and Occupational Therapists. The team provides support with the Acute Hospital as well as transfers of care within the Virtual Homefirst Hub with our partners, providing Care Act Assessments , Carers assessments, Safeguarding enquiries and reviews. With our integrated working, you'll enjoy variety in your day and the opportunity to build strong working relationships with professionals across the county council, NHS and other partners.

Our exciting model of care - Living Well changes the conversation for social work to provide better outcomes and aligns with our vision to support people to be independent, resilient and well. Talking about what they can do, listening and connecting to enable early intervention in the community and avoid where possible admission to an acute setting.

There will be an expectation that you will work from James Paget Hospital to enable you to speak directly with the adults we are working within a timely manner, but we do offer some flexible hybrid working Our technology platform and equipment are first class for you to connect and collaborate remotely.

East Norfolk is an area with award winning beaches and a mix of rural towns and villages. Close to the border of Suffolk, and with easy access to the county of Essex and beyond makes it a perfect place for easy travelling to and from the area.

Parts of both Norfolk and Suffolk combine to make up this locality. Large swathes of the Broads National Park run from north to south, with windmills dotting the skyline and huge beaches forming the coastline, including the seaside towns of Hemsby, Caister-on-Sea and Southwold. Inland, Beccles and Bungay are beautiful historic market towns.

The largest Norfolk town in this locality is the popular seaside resort, market town and maritime port of Great Yarmouth, 21 miles east of Norwich and easily accessible by road or rail. Lowestoft in Suffolk is the most easterly town in England, 28 miles from Norwich. It is also a seaside resort, market town and maritime port with direct rail access to Norwich and also to London via Ipswich.
