ZOE International is a faith-based Christian organization whose mission is to reach every person with the gospel and rescue every child from human trafficking. The Social Worker is responsible for ensuring individualized, trauma informed services are provided for the residential and clinical needs of all residents. Under the supervision of the Clinical Manager, the Social Worker will be responsible for partnering with the youth's Child and Family Team, service providers, and other agency partners to provide case management and coordinate delivery of residential and mental health services.
Responsibilities:
- Completes Needs and Service Plans monthly
- Conducts Intake Assessment for admission to STRTP
- Completes a transfer plan
- Ensures provision of or access to core services and supports
- Coordinate and facilitate the Child and Family Team meetings on behalf of residents
- Understands and meets all regulatory requirements for documentation, assuring timely completion and entries in the residents' records to include, but not limited to: the resident's assessments, a care plan to address strengths, needs, abilities, and preferences, substantiation of interventions, progress toward, and/or completion of goals, and transition summaries
- Monitors, supports, and assists residents in developing and/or maintaining skills needed to achieve the Mental Health Treatment Plan goals
- Intervenes via telephone or in person with residents
- Promotes the on-going participation of the client, family, and DCFS Social Worker, when applicable, in discussions of the client's mental health treatment and progress.
- Identifies and addresses gaps in mental health service provision
- Communicates information regarding progress of the client to the appropriate partners, including the treatment team and family as appropriate
- Builds rapport with residents in order to ensure all health, educational, religious, recreational, social, emotional, and psychological needs of each child are met
- Demonstrates awareness, sensitivity, and competence in serving a culturally diverse population
- Actively participates in ongoing training as needed in order to meet all certification standards and credentialing policies
- All other relevant duties as assigned by the supervisor or his/her designee as needed
- A Master's Degree from an accredited or state-approved graduate school, as defined by the Department of Education, in social work or social welfare, marriage, family and child counseling, child psychology, child development, counseling psychology or social psychology.
- At least 21 years of age
- Completion of at least three semester units or 100 days of field practice or experience in a public or private social service agency at the Master's Degree level.
- At least nine-semester units of coursework related to children and families or 18 months of experience in working with children and families.
- At least three semester units in working with minority populations; six months of experience in working with minority population; or six months in-service training in working with minority populations within the first year of employment as a condition of employment.
- At least three semester units in child welfare, or two years' experience in a public or private child welfare social services setting
Other Requirements:
- Current CPR and First Aid certification
- Valid California Driver's license and a good driving record (Department of Motor Vehicle printout required.)
- Criminal record clearance
- Willingness to sign and adhere to ZOE International's Code of Conduct, Child Protection Policy, and other policies and procedures
Additional Information:
Verbal fluency in Spanish is preferred but not required
