As a Mental Health Link worker your role will be to refer and connect patients with mental health illnesses to services and community groups and third sector organisations to improve their wellbeing and enable them to live a fulfilling life.
Taking a holistic approach, you will work with people to help them to access information, support and activities that they will benefit from and enjoy.
You will need experience of working with people with various mental health illnesses in a holistic way. A positive ‘can-do’ attitude, high levels of resilience, excellent communication skills and the ability to motivate, empower and support people to achieve their goals.
Key responsibilities will include supporting clients referred by GPs to the Mental Health Integrated Network Team, (MINT) to access locally-based community services to signpost and assist with their mental health recovery.
A key part of the role is to develop a live register of assets in the community that could be useful to people on the caseload who would benefit and to work with other potholders on a system-wide asset-based register.

Mental Health Link Workers are based within the Mental Health Integrated Network Teams (MINT) to provide a person-centred service that is responsive to the needs and interests of people living within the community covered by the local Primary Care Network (PCN). The role of the link worker is to support clients referred by GPs to MINT to access locally-based community services to signpost and assist with their mental health recovery. A key part of the role is to develop a live register of assets in the community that could be useful to people on the caseload who would benefit and to work with other post-holders on a system-wide asset-based register. This role is a leadership role tasked with developing the link worker role in the MINT (Mental Health Integrated Network Teams) and supervising the link workers in post.
