As a Preparing for Adulthood (PfA) Co-ordinator, you will support children and young people aged 14–25 as they prepare for and transition into adulthood. Starting from Year 9, you will work with them, their families and involved professionals to explore aspirations and plan across key areas: education, employment, independent living, health, and community inclusion. You will ensure these goals are accurately reflected in Education, Health and Care Plans (EHCPs) and other support arrangements.

You will also play a vital role in joining up services across children's and adult pathways, minimising disruption and promoting smooth, consistent transitions. Collaboration will be central to your work, as you partner with Children's Social Care, SEND teams, education providers, Adult Social Care, health professionals, and voluntary organisations.

Your expertise will help colleagues and partners adopt a joined-up, outcome-focused approach. Strong record-keeping and case management will ensure progress is monitored carefully and any risks are addressed.

Due to the nature of the role, applicants must hold a recognised Social Work qualification and be registered with Social Work England.

This role is suitable as a job share opportunity.

As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C1: Can express themselves fluently and spontaneously, almost effortlessly. Only a conceptually difficult subject can hinder a natural, smooth flow of language.

Hybrid working arrangements are available for this role. Each role at CYC is different and your working arrangements will be determined based on the requirements of the role and in collaboration with you and your needs as part of the recruitment process. Your contractual location will be the designated council office for your team.
