Job summary

The role of the Care Coordinator is to act as a central point of contact to help patients with mental health needs navigate and access a variety of support services. They are responsible for creating care plans, coordinating between different healthcare professionals, advocating for the patient, and connecting them with resources like social care, housing assistance, and community groups. The Care Coordinator would monitor progress and focus on goals that are important to the patient. They will also support family members where appropriate and ensure that the carers are receiving support if a need is identified.

Main duties, tasks & skills required

Conduct thorough assessments of a patient's physical, mental, and social needs.

Create and manage personalised care plans that outline specific goals and the services required to achieve them.

Prioritise services based on the individual's needs and monitor progress, making adjustments as necessary

Act as a central point of contact, coordinating all aspects of a patient's treatment plan and ensuring services are delivered in a timely manner.

Facilitate communication between various healthcare providers, such as doctors, psychiatrists, therapists, and social workers.

Help patients navigate the mental health system by connecting them with the right services at the right time

Provide patients and their families with information about their condition and help them understand treatment options.

Assist patients with practical matters, such as preparing for appointments and managing their medication if applicable.

Advocate for patients, ensuring their needs are met and helping to facilitate shared decision-making
