YOU MUST BE ABLE TO DRIVE AND HAVE A CAR FOR THIS JOB ROLE Role Description This is a full‑time, on‑site role located in Letchworth Garden City, Hertfordshire. The Care Coordinator will be responsible for managing and scheduling care services, ensuring the efficient delivery of high‑quality care to clients. Key Responsibilities Answer incoming telephone enquiries. Maintain communication with care and support workers and service users regarding rotas. Build and maintain relationships with other professionals we work alongside. Identify gaps in the rota and source new packages of care via e‑brokerage or privately. Process new client referrals and arrange assessments and start dates. Prepare appropriate documentation for Field Care Supervisors. Allocate and template Care & Support Workers to all client visits, ensuring all visits are covered. Shared on‑call duties in a 1 in 3 weekend rota. Ensure all clients and Care & Support Worker documentation and electronic records are kept up to date. Assist with team meetings and event organisation. Authorise and manage annual leave. Manage an area of care and support workers and service users. Work closely with the Field Care Supervisor. Maintain compliance with all office systems and procedures. Produce reports and management information as required. Be part of the on‑call team (early morning, evenings and weekends). Record and address issues that arise ‘Out of Hours’. Manage emergencies when on‑call (support the Care Assistant or client until emergency services arrive during mornings, evenings and weekends). Complete weekly manager reports for care manager. Keep our CRM (Leads) up to date on People Planner. Work towards a 3‑week lead time. Other Key Information Potential start date – as soon as possible. Hours: Core Monday‑Friday 08:30 am–17:30 pm. On‑call hours: rota basis including mornings, evenings and 1 in 3 weekends. Salary: depending on experience. Benefits Company events. Company pension. Free flu jab. Free or subsidised travel.
