Physiotherapist - Neighbourhood Rehabilitation Service Job Summary: Provide rehabilitation to help people reach their potential and live well in their community by working with the Neighbourhood Rehabilitation Service (NRS) which includes Intermediate Care, Long Term Conditions and Crisis pathways across Barnsley Community. Deliver high‑quality therapy intervention in line with Trust values, prevent unnecessary hospital admissions, promote functional independence and support discharge. Work within a multi‑professional team, manage a varied caseload, and provide high‑quality rehabilitation in patients’ homes and local care homes. You will assess, manage and provide treatment for individuals with complex and/or multiple needs, working closely with the MDT and supervising other staff when required. Car driving licence and access to a vehicle are essential. Responsibilities Manage a clinical caseload with autonomy. Demonstrate knowledge, experience and skills for patients with complex/long‑term conditions, falls, orthopaedics and general mobility problems. Undertake comprehensive assessment and provide therapeutic diagnosis using advanced investigative skills and specialist listening. Document and implement agreed treatment plans, monitor and evaluate treatment outcomes, and adjust plans as necessary. Develop programmes of care to support discharge and contribute to clinical leadership/supervision to facilitate a culture of learning and reflective practice. Communicate effectively and maintain excellent interpersonal skills. Ensure compliance with clinical governance, infection control policy, and all relevant NHS standards. Work flexibly, including evenings, weekends and bank holidays as required. Qualifications & Experience BSc in Physiotherapy or equivalent (Diploma). HCPC Registration. Experience of risk assessment, moving and handling, and the safe use of specialist equipment. Integrated multi‑disciplinary team working experience. Experience in a specialist clinical area directly related to the post, and/or supervision of assistants/students. Substantial post‑registration rotational experience across service areas such as respiratory care/medicine, neurology and older peoples’ service. Evidence of ongoing CPD and commitment to maintaining specialist knowledge of evidence‑based practice. Physical & Personal Attributes Ability to undertake the duties and demands of the post with an acceptable sickness record. Excellent communication and organisational skills. Capacity to work independently and as part of a team; ability to teach and supervise. Current driving licence and access to a car during working hours. Training & Development CPD activities and portfolio updating to reflect personal development. Involvement in audit, research, quality improvement, and relevant postgraduate study/course. Participation in the Trust’s appraisal system and in-service training programmes.
