We would like to hear from you We are looking to appoint a Physiotherapist to join the Neighbourhood Rehabilitation Service (NRS) which includes Intermediate Care, Long Term Conditions and Crisis pathways, working across Barnsley Community. NRS aims to deliver high‑quality therapy intervention in line with trust values. Prevent unnecessary hospital admissions & facilitate discharge by providing support & rehabilitation. Provide multi‑disciplinary rehabilitation & support to promote functional independence. Responsibilities Work within a multi‑professional team and hold a varied caseload providing therapy input & high‑quality rehabilitation to people within their own homes & local care homes. Assess, manage & provide treatment to individuals with complex and/or multiple needs, working closely with the MDT and providing supervision/support to other staff. Manage a clinical caseload with autonomy. Demonstrate knowledge, experience & relevant clinical skills for patients with complex/long‑term conditions, falls, orthopaedics & general mobility problems. Undertake comprehensive assessment, provide therapeutic diagnosis using investigative, advanced analytical skills, specialist listening, physical & observational skills, interpret clinical findings & implement agreed treatment plans. Monitor & evaluate treatment to measure progress/outcomes to ensure effectiveness of interventions & service delivery. Develop programmes of care to support discharge. Contribute to clinical leadership/supervision facilitating a culture of learning/reflective practice. Have high level communication & excellent interpersonal skills. Ensure compliance with clinical governance, including Infection Control policy/procedures. Have the ability to work flexibly including evenings, weekends & bank holidays. Qualifications & Requirements Qualified Physiotherapist with relevant clinical experience. Ability to work independently and as part of a multidisciplinary team. Strong analytical, communication and interpersonal skills. Access to own vehicle and valid driving licence (required). Compliance with clinical governance and Infection Control procedures.
