Rehabilitation Support Worker – Neighbourhood Rehabilitation Service South West Yorkshire Partnership NHS Trust Closing date: 12 November 2025 Are you committed to delivering compassionate, person‑centred care that empowers individuals to remain independent? Join our dynamic Neighbourhood Rehabilitation Team as a Rehabilitation Support Worker (RSW) & make a real difference in your community. You will work under the guidance of registered healthcare professionals to deliver high quality, person‑centred care on the Intermediate Care pathway. You will work both independently & collaboratively supporting individuals in their own homes & care homes. Deliver delegated care tasks including personal care & medication support. Promote independence through mobility practice, encouraging participation in exercise programmes/therapy plans & assisting with meal preparation & daily living skills. Desirable to work in the NHS, be motivated & flexible, and work a shift pattern including weekends & bank holidays. This is a community role; a current driving licence & access to a vehicle to undertake travel in connection with the duties of the post is essential. Visa sponsorship considerations: Visa sponsorship under the Health and Care Worker visa route can only be considered if the prorated salary meets the minimum threshold of £25,000. For Band 3 roles, only full‑time positions at the top of the pay scale meet this threshold. Therefore, sponsorship may not be possible depending on salary increment placement and working hours. Main duties of the post As a Rehabilitation Support Worker you will: Provide high standards of safe & compassionate care that maximises patients’ rehabilitation potential. Work in close liaison with the MDT, maintaining excellent communication and time‑management skills. Assist with initial assessment & care plan development/implementation. Participate in training, education & clinical supervision to develop skills & knowledge and apply continuous reflection on own practice. Work independently and as part of a team. Demonstrate strong communication and observational skills. Maintain accurate clinical records. Support safe use of equipment such as hoists, walking aids & pressure cushions. Provide information to support your application relating to your skills & personal qualities, telling us how you meet the person specification criteria providing examples. Job responsibilities (summary) The post holder will: Perform delegated routine interventions and care tasks under the direction of registered healthcare professionals. Support appointments with service users in their homes and the community (including care homes), mainly working alone under direction or with other team members. Assist with personal care tasks including bathing, toileting, dressing and administering pre‑dispensed medication. Provide assistance with meals and drinks, keeping a record of all activities. Support service users in following therapy care plans to promote rehabilitation, including bed exercises, stair practice & balance exercises. Deliver care within established policies, procedures & guidelines. Encourage and motivate service users to develop daily living skills and undertake mobilisation exercises. Document care and progress accurately, report concerns & update the registered professional & team. Maintain a safe environment, respond appropriately to emergencies & report incidents using Datix. Participate in a flexible rostering arrangement, providing 7‑day care including unsocial hours as appropriate. Person Specification Experience In providing personal care and related procedures for those with health/social needs. In working in a caring environment at a Band 2 or equivalent level of competence. In dealing with the public. In working as an effective team member. Demonstrated own duties. Ability to work effectively, professionally & under own initiative or as part of a multidisciplinary team. In demonstrating initiative, decision‑making & prioritisation skills. Evidence of a commitment to user/carer involvement. Special Knowledge/Skills Understanding of importance of customer care skills. Basic IT/keyboard skills, good administration & organisation skills. Understanding confidentiality. Resilient & able to remain calm under pressure. Enthusiastic, able to work flexibly on a 24h shift pattern as required. Demonstrating duty to new / less experienced staff. Non‑judgemental, empathetic manner. Ability to identify changing presentation & adapt quickly. Effective interpersonal & communication skills. Work as part of a team. Engage fully in the development programme if identified as part of the post. Use observation skills to identify potential service user, environmental risks. Safe decision‑making skills commensurate with role. Language skills other than English. Understanding of people who have experienced ill health. Qualifications GCSE A‑D or grade 3‑9 in English & Maths or equivalent. Level 2 qualification or demonstration of competencies at level 3. Care Certificate or required to achieve within 12 weeks of commencement. Apprenticeship level 3 or equivalent (e.g., RQF NVQ level 3 in Health & Social Care). Experience of the speciality area. Ability to commute between sites or hold current driving licence & access to a car during the working day (reasonable adjustments considered if unable to drive due to a disability). Training Must undertake all Trust mandatory training requirements, including the care certificate & subsequent refresher training relevant to the area. Must undertake development & training (including a level 2 qualification in health & social care if eligible). Excellent knowledge of service user safety (e.g., prevention & management of violence & aggression, moving & handling). Willing to undertake any in‑house training (e.g., medication training or specialist training). Physical Attributes A satisfactory sickness record over previous 2 years (subject to fairness & equality of opportunity). Able to fulfil Occupational Health requirements for the post (reasonable adjustments if necessary).
