Whether it be supporting a team member with new skills or attending to a patient and advising their family, your role as a Community Nurse provides you with the opportunity to deliver a high standard of care to patients in their homes alongside developing your leadership skills supporting your team. Could you be our Community Nurse looking for a new challenge? As a Registered Nurse you will contribute to leading a community nursing team within a multi professional environment. The role requires travel around the local community. This is only a fixed term post to cover the maternity leave for 10 months. You will need to have a full driver’s licence and access to a car. The service covers 8am - 8pm working 7 days per week. The shift patterns are 8am - 4pm, 9am-5pm, 10am - 6pm, and 12pm - 8pm fulltime including weekend working. Responsibilities Provide wound care, catheter care, and palliative care Administer IV drugs, medication and injections Manage diabetes and other diverse nursing skills Work weekends, late shifts and bank holidays pro rata; late shift covers until 8pm Newly qualified staff will have access to a preceptorship program and specific clinical upskilling training along with PNDs support. Two weeks of trust induction and two weeks of local team induction will be provided, shadowing DNs, TVNs, CNS etc. Team & Collaboration As a member of our community multi-disciplinary team, you will work within an integrated team of District Nurses, Specialist Nurses, Community Matrons, Therapists, and Health Care Assistants to support the provision of high‑quality evidence‑based care. You will also collaborate with Local Authorities, Integrated Care Networks, GP’s, and Acute sector providers to facilitate care pathways that support early discharge and prevent hospital admission.
