The Triage ANP role is integral to ensuring that all patient requests into the practice are clinically assessed and prioritised accordingly. The post holder will provide self-care advice appropriate to the patients needs or allocate them to the most appropriate practitioner or service. There may be occasions where the triage ANP would need to see some of the patients face to face.Main duties of the jobWorking as part of the triage team, the post holder will provide clinical triage of all online consultations, telephone calls and home visit requests where needed.The post holder will identify the clinical need and urgency to provide a time frame for intervention, allocating them to the most appropriate clinician or service to meet the needs and will provide advice and signposting as required.There may be, on occasion, a requirement to see some patients face to face.About usAshby Turn Primary Care Centre has a patient list size of approx. 12,400.We are a friendly, inclusive practice with 3 GP Partners and an extensive clinical and admin team, including GP registrars.Job responsibilitiesThe post holder will utilise clinical triage and assessment skills, whilst working within the Scope of Professional Practice and adhering to local policies/guidelines.
