Overview This is an exciting opportunity for a qualified nurse/paramedic practitioner to join a team of urgent care practitioners in North Lincolnshire. The successful candidate will be responsible for the delivery of urgent care services in the North Lincolnshire community, providing an integrated service model intended to reduce waiting times for patients accessing unscheduled care if medically housebound, at their usual place of residence to reduce or avoid unnecessary A&E attendance, hospital admission, and ambulance conveyances by accessing the right care at the point of need. This gives the patients a No wrong door approach at the time of crisis. The team forms part of the delivery of two UCR services in North Lincolnshire. The post holder will have opportunities to further develop their skills in other areas such as Virtual ward. Responsibilities To work as an autonomous practitioner, providing clinical care (including telephone triage, diagnosis, treatment, referral, review or discharge) as appropriate, utilising a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions.
