Diabetes Specialist Nurse – Midlands Partnership University NHS Foundation Trust Job Summary The Community Diabetes Specialist Service is looking to recruit a qualified nurse with experience in diabetes management for the post of Community Diabetes Specialist Nurse. The post holder will join the current team of specialist nurses, educators, and administration staff. The aim of the service is to provide specialist diabetes support and interventions in the community and primary care. You must be a qualified nurse with at least 5 years post registration/graduate experience, hold an accredited course in diabetes (preferably a post‑graduate diploma in a diabetes‑related field) and demonstrate experience and knowledge in education, teaching and mentorship for health care professionals and patients. Nurse prescribing and experience of working in a specialist diabetes role is essential. Experience of carbohydrate counting, insulin‑pump management and the delivery of BERTIE structured education would be advantageous, although training will be provided for the right candidate where service delivery is required. Applicants must be able to work independently as well as part of the multi‑disciplinary team. The role involves nurse‑led clinics, multi‑disciplinary clinics and home visits, and a key role in supporting and educating patients, community and practice staff to improve the quality of diabetes care. Be an expert diabetes practitioner with detailed professional and clinical knowledge. Be an advanced clinical practitioner with experience managing Type 1 and Type 2 diabetes, including insulin initiation and titration, management of patients requiring GLP‑1 therapy, insulin pump management (South Division only), carbohydrate counting and structured education for people with Type 1 diabetes. Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community. Be skilled in assessing complex diabetes specialist care and support needs and in developing, implementing and evaluating care. Have supervisory, training and clinical leadership responsibilities where appropriate and some responsibility for team performance and service development/improvements. Demonstrate initiative and be creative in finding solutions to problems. Work with the Clinical Lead nurse and Operational Lead to undertake delegated tasks in team management and service improvements.
