Clinical Pharmacist – Stafford Central PCN (Gnosall Health Centre) Location: Gnosall Surgery, Brookhouse Road, Gnosall, Stafford, ST20 0GP, UK. Contract: Permanent – Part‑time (30 hrs per week). Job Summary Expand the Pharmacy Team by providing dynamic clinical support, managing targeted chronic disease areas, performing structured medication reviews, and advising on prescribing, medication promotion, and clinical optimisation within a multidisciplinary primary care environment. Key Responsibilities Carry out Level 3 medication reviews for patients, focusing on complex poly‑pharmacy and chronic disease management. Manage repeat prescribing re‑authorisation, review medicines at renewal dates, and ensure appropriate monitoring tests. Reconcile medicines post‑hospital discharge and liaise with community pharmacists to secure continuity of care. Contribute to reductions in medicine‑related hospital admissions and readmissions through targeted reviews. Develop referral processes between primary, community, and hospital pharmacy teams and promote repeat dispensing services. Audit and implement medication safety measures, including drug withdrawals and MHRA alerts. Apply NICE and other evidence‑based guidelines to optimise prescriptions. Deliver clinical support to practice staff on prescribing queries, acute requests, and system safety. Participate in clinics such as anticoagulant monitoring, high‑risk drug monitoring, and multi‑morbidity reviews. Provide education and training to the primary health team on therapeutics and medication optimisation. Support the Quality and Outcomes Framework and enhanced services in the PCN. Maintain clear, accurate records in compliance with legislation and practice policies. Person Specification – Essential MPharm (4 years) or equivalent. Membership of the General Pharmaceutical Council of GB (pre‑registration training). Prescribing qualification and postgraduate clinical pharmacy qualification. Experience in clinical pharmacy, working with multidisciplinary teams, and clinical audit. Good knowledge of drug use, pathophysiology, medicine management, NHS drivers, and UK law & ethics. Strong communication, presentation, organisational and time‑management skills. Leadership ability, ability to work independently or in a team, and computer literacy (Microsoft Office, ePACT). High responsibility, initiative, commitment to CPD, flexibility in working arrangements, and professional appearance. Person Specification – Desirable Membership of the Royal Pharmaceutical Council of GB. Experience in a PCN, at least 5 years post‑registration with 2 years in primary care. Smoking‑cessation advisor qualification. Teaching/training experience for staff. Financial management or budgetary reporting on medicines. Experience with EMIS Web GP system, project management skills, and IT qualification (e.g., ECDL). Skills Clinical knowledge application across varied practice settings. Excellent written and verbal communication. Presentation and organisational skills. Effective time management and prioritisation. Teamwork and liaison with other professionals. Computer skills and ePACT proficiency. Diplomatic, supportive of colleagues, and leadership traits. Conflict handling, professionalism, discretion in sensitive situations. Proficiency in VDU use.
